Parent/Guardian: Children will be registered by the adult who is attending church with them on Sundays. If this adult is not a parent, we will consider them to be the child's guardian and they will be contacted by ministry staff in the event of incident or emergency. It will also be understood that this adult will be at the church facility and easily accessible while the child is in Kids Church.
I hereby declare that I will not hold Bethel, nor any member of it's organization responsible for any accident or injury incurred while participating in Bethel activities.
Supervision: Children will be in the care of their parents/guardians until they are released from the adult service for their class. Prior to this time Bethel is not responsible for their supervision or care. Should a child need to use the washroom, parents are encouraged to accompany them to the washrooms.
In the event of an emergency, and the contact persons listed below cannot be reached, I authorize the Bethel as an agent for myself to consent to treatment and hospital care advised and supervised by a physician.
I give permission to Bethel to utilize any photos taken during camp for photo collages, updates shared with parents, and future church promotions only.
Dismissal: Parents are asked to come to the class quickly following the end of the adult service. They will be asked to show their security label before children leave the room.
Bethel does not provide transportation to or home from Kids Church.
