Information & Consent

Parent/Guardian: Students will be registered by the adult who is attending church with them on Sundays. If this adult is not a parent, we will consider them to be the child's guardian and they will be contacted by ministry staff in the event of incident or emergency. It will also be understood that this adult will be at the church facility and easily accessible while the student is in Second Sunday Group.

I hereby declare that I will not hold Bethel, nor any member of it's organization responsible for any accident or injury incurred while participating in Bethel activities.

Supervision: Students will be in the care of their parents/guardians until they are released from the adult service for their class. Prior to this time Bethel is not responsible for their supervision or care.

In the event of an emergency, and the contact persons listed below cannot be reached, I authorize the Bethel as an agent for myself to consent to treatment and hospital care advised and supervised by a physician.

I give permission to Bethel to utilize any photos taken during camp for photo collages, updates shared with parents, and future church promotions only.

Bethel does not provide transportation to or home from Second Sunday.

I have read this form and accept the conditions of registration. I give my student (as listed below) to participate in the activities of Bethel, including the various sports, games, crafts, & other program events under the supervision of Bethel Staff.
